Terms and Conditions – Appointment Cancellation & Rescheduling Policy

Cancellation & Refund Policy

  • Cancellations made at least 48 hours prior to your scheduled appointment are eligible for a refund.

  • Cancellations made within 48 hours of the appointment are eligible for a 50% refund.

  • No refunds will be issued for same-day cancellations or no-shows.

  • All cancellations or changes must be made by phone at 404-927-8554.

Rescheduling Policy

  • Appointments may be rescheduled at least 48 hours in advance at no charge.

  • Rescheduling within 48 hours of the appointment will result in a $50 fee.

  • No-call / No-show appointments will forfeit payment and require full payment to rebook.

Identification Requirements

  • All individuals 18 years and older must present valid, unexpired government-issued photo identification at the time of service.

  • Acceptable forms of identification include:

  • State-issued ID

  • Driver’s License

  • Passport or Passport Card

  • Permanent Resident Card (Green Card)

  • Failure to provide valid identification may result in cancellation without refund.

Test Results & Participants

  • Test results are released only to the individual who purchases the test.

  • Additional participants may incur extra fees depending on the service type.

  • Please contact us prior to your appointment for pricing regarding additional participants.

SMS/Text Messaging Terms

  • By providing your phone number and opting in, you consent to receive SMS messages from Level Up Testing.

  • Messages may include appointment confirmations, reminders, booking instructions, service updates, and customer support communication.

  • Message frequency may vary depending on your interaction with our services.

  • Message and data rates may apply depending on your mobile carrier.

  • Consent to receive SMS messages is not a condition of purchase.

  • You may opt out at any time by replying STOP.
